It’s authentic Victorian, so yes, the background is fake. The skinny guy is George Moore, and he was a 7′2″-tall circus giant. The fat man’s name is Fred Howe and he supposedly weighed 435 pounds. Both were circus performers and they staged a number of pictures like this. Here’s another:
